NMN and NAD+: What Clinical Evidence Says About the Anti-Aging Supplement

Nicotinamide mononucleotide (NMN) has become one of the most discussed anti-aging supplements. The 2024 meta-analyses are the first to systematically assess the evidence: NMN does raise NAD+, but functional effects in humans proved surprisingly modest.

Why does NAD+ matter to the cell?

Nicotinamide adenine dinucleotide (NAD+) is a coenzyme present in every cell of the body. Its role spans three critical processes: ATP production (mitochondrial respiration), repair of damaged DNA (via PARP enzymes), and regulation of gene expression through the sirtuin protein family. It is sirtuins that link NAD+ to longevity at the molecular level — they govern inflammation, stress resilience, and metabolic flexibility.

The problem is that NAD+ concentration in tissues declines with age. Animal studies have convincingly shown that restoring NAD+ levels improves health and extends lifespan. This observation gave rise to the hypothesis that NAD+ precursors — nicotinamide mononucleotide (NMN) and nicotinamide riboside (NR) — might slow age-related processes in humans.

How NMN raises NAD+ — and what that means

NMN is a direct biochemical precursor to NAD+. After intestinal absorption, NMN is converted to NAD+ through a series of enzymatic reactions. This mechanism has been confirmed in both cell cultures and clinical trials in humans.

The most reproducible result across all trials conducted is precisely the rise in blood NAD+. In the randomized double-blind study by Igarashi et al., blood NAD+ levels increased by week 4; a similar result was recorded in most trials using doses of 250 mg/day and above. This is the only action of NMN that can be considered established with sufficient confidence.

The question science has not yet resolved: what does this rise in blood NAD+ mean for human health? Blood levels and tissue levels are different things — and it is tissue NAD+ that determines biological effects.

NMN consistently raises blood NAD+ — that is established. What exactly this increase does for the human body remains an open question.

What the 2024 meta-analyses showed

By 2024, enough randomized trials in humans had accumulated to conduct meta-analyses — the most reliable tool for synthesizing evidence.

Metabolic outcomes. Chen, Zhou, and co-authors (Current Diabetes Reports, 2024) analyzed 8 RCTs involving 342 middle-aged and older adults. Doses ranged from 250 to 2000 mg/day, duration from 14 days to 12 weeks. Result: NMN had no significant effect on fasting glucose, insulin, glycated hemoglobin HbA1c, HOMA-IR index, or lipid profile. An initially detected reduction in HOMA-IR was not confirmed in sensitivity analysis.

Physical function. Wen and co-authors (Cureus, August 2024) synthesized 10 RCTs with 437 participants (mean age 58 years, range 35–81). Doses: 150–1200 mg/day. Muscle mass and maximal strength did not improve significantly in any included trial. However, some studies recorded localized signals:

  • Walking speed: Igarashi et al. — significant improvement (p=0.002)
  • 6-minute walk distance: Yi et al. — dose-dependent improvement (p<0.05)
  • 5-chair stand test: Kim et al. (p=0.04)
  • Aerobic power at ventilatory threshold: Liao et al. — dose-dependent improvement (p=0.03)

These signals are real, but come from individual small trials. Their reproducibility and clinical significance require confirmation in larger studies.

Limitations of the evidence base

All trials included in the meta-analyses share a number of common weaknesses. Sample sizes are small — mean participants per trial around 40–50. Duration does not exceed 12 weeks, whereas age-associated NAD+ changes developed over decades. Most participants are healthy older adults, meaning results extrapolate poorly to younger cohorts.

An additional problem: different methods of measuring NAD+. Levels in blood, in muscle biopsies, and in peripheral blood cells are different compartments, and the correlation between them is not always clear. When trials report "NAD+ elevation," it matters which compartment is being measured.

On safety: of 437 participants, 8.2% reported adverse events (Wen et al., 2024), none classified as serious. Safety data beyond 20 weeks of intake are absent.

What this means in practice
  • NMN raises blood NAD+ — that is established. Treating this as direct proof of health benefit is premature.
  • Metabolic effects (glucose, insulin, blood lipids) in healthy individuals are not clinically proven — taking NMN "for metabolism" is not evidence-based.
  • Signs of improved functional mobility in older adults exist, but come from individual small trials: the signal requires verification.
  • If considering NMN supplementation, studied doses are 250–900 mg/day as a single or divided dose; trial durations did not exceed 12 weeks.
  • The short-term safety profile is acceptable; long-term data are absent — a risk that should be factored in.
  • Foundational interventions — adequate sleep, resistance training, high-fiber diet — have established effects on metabolic health. NMN does not replace them.
